FOR THIS INFORMATION TO BE CONSIDERED, THE NOMINATOR MUST COMPLY WITH ALL OF THE INSTRUCTIONS. INCOMPLETE PACKETS WILL BE REMOVED FROM CONSIDERATION.
Nomination Steps
1. Intent to nominate - Use the INTENT TO NOMINATE FORM button to the left and download or print the form. Fill in all the blanks. The deadline for submitting the intent to nominate form is listed at the bottom of the form. This form will notify the selection committee to expect a NOMINATION PACKET for the intended teacher. The Intent to Nominate form should be returned by December 16, 2011.
2. Nomination Packet - This packet will contain the following items:
NOMINATION FORM - Use the NOMINATION FORM button to the left and download or print the form. Fill in all the blanks. The deadline for submitting the nomination packet is listed at the bottom of the nomination form.
NOMINATOR STATEMENT - This statement is from the person nominating an AISD classroom teacher. It should reflect your reasons for the nominations.
NOMINEE STATEMENT - This statement will come from the teacher being nominated. It should express what the nominee feels are the characteristics of an excellent AISD classroom teacher.
LETTERS OF RECOMMENDATION - In addition to the statements listed above, your packet must include two (2) and only two (2) letters of recommendation. These letters may come from fellow teachers, supervisors, parents, students or any source that might be important to the nomination with the exception of family members. If more than two (2) letters of recommendation are submitted, the additional letters of recommendation will be discarded.
ADDITIONAL INFORMATION - Please include a copy of the nominee's daily teaching schedual, including room numbers, class times, planning periods, and any special activity periods. High school teachers should indicate A and B schedules. Also, student teacher information should be included.
ONLY ONE (1) COPY OF EACH DOCUMENT IS REQUIRED.
IN THE EVENT A TEACHER IS NOMINATED MORE THAN ONCE, ONLY THE FIRST PACKET RECEIVED WILL BE CONSIDERED.
PLEASE DO NOT INCLUDE ART WORK, HARD BOUND COVERS OR ANY OTHER MATERIAL NOT SPECIFICALLY REQUESTED ABOVE.
